They may experience relaxation or euphoria. They may cry or laugh for varying periods. They may, very exceptionally, have an epileptic fit. All these things are effects that can be produced by stimulating parts of the limbic system, especially the cingulate cortex (euphoria, fits) and the amygdala (fear). It is therefore possible that some of the more unusual kinds of acupuncture effects may be mediated by the limbic system (Campbell, 1999). The role of neurotransmitters A large number of neurotransmitters is known to exist but the relevance of most of them to acupuncture is uncertain.

If a patient is thought likely to faint, they should be placed in a suitable position so that they don’t slide onto the floor. Sweating Patients receiving acupuncture may sweat profusely. Often this may be a manifestation of anxiety, but not always. Some people routinely sweat when needled, especially in the hands, even though they don’t appear to be anxious. Convulsions Occasionally patients receiving acupuncture have an epileptic fit. This is certainly uncommon: in over 25 years’ practice I have never seen a case, but it has been reported a number of times.

There has been much controversy about the relevance of the endogenous opioids to acupuncture analgesia and the position is still unclear. One paradoxical finding is that stimuli that produce release of opioid peptides also cause release of an antagonist to them: cholecystokinin. It has been suggested that the ineffectiveness of acupuncture in about 20 per cent of the population may be related to higher than usual secretion rates of cholecystokinin. Serotonin has been shown in some experiments to increase the effectiveness of acupuncture analgesia.
